What counts as a Proof of Address?

Under FDA guidelines, plasma donation centers must require donors to provide proof of a permanent residential address where they can be contacted for follow-up for at least 8 weeks after their donation. Homeless shelters, hotels, missionaries, recovery facilities, and Airbnbs are considered temporary residences and do not meet this requirement. This regulation is part of the broader framework to ensure the safety and traceability of blood and plasma donations. PO Boxes can be used as a secondary mailing address but must accompany proof of residence.

We can accept the following types of items for your proof of address:
  • Mailed letters, documents, or packages dated or post marked within 60 days (e.g., utility bill, credit card/bank statement, paycheck stub, greeting card, etc.)
  • Valid Photo ID that includes your current residential address
  • Current homeowner’s or renter’s insurance policy or statement
  • Current automobile insurance policy or statement
  • Current mortgage statement
  • Voter or vehicle registration card
  • Current lease or rental agreement
  • House deed
  • Current College Enrollment document
  • Current housing accommodations/assignment from college or military base (e.g. Dorm Assignment, Base Barrack Assignment)
