What do I need to provide for Proof of SSN?
We can accept the following types of documentation for your social security number:
- Social Security Card, Social Security Card Stub, or receipt from SSA for card replacement
- SSN from Social Security Administration
- Social Security Benefit Statement
- Medicare Card
- Employment Authorization Documents
- Military records with SSN (e.g., DD-214)
- Federal program documentation (e.g., SNAP or WIC Certification)
- Selective Service Registration Card
- Department of Human Services Documents
- Border Crossing ID (e.g., B1/B2)
- Tax document (e.g., W-2, 1099, and SSA 1099,)
- Loan Application Document from creditor
