To ensure that your first visit goes smoothly, you will want to ensure you have a few pieces of documentation with you. The three forms of identification you'll need to bring to your first appointment are a Proof of Identification, Proof of Address, and Proof of Social Security Number. You may also want to ensure you have a complete list of the medications you are currently taking.
