Your First Visit

To ensure your first visit goes smoothly, there are a few pieces of documentation you will need to bring. Join Parachute requires these three forms of identification to complete registration on your first appointment: Proof of Identification, Proof of Address, and Proof of Social Security Number. You may also want to ensure you have a complete list of the medications you are currently taking. 

Proof of Identification

You will need a valid, unexpired form of a photo ID. Acceptable photo IDs include:

  • Driver’s License
  • Temporary State or license ID
  • State-Issued ID / Age of Majority Card
  • Learner’s Permit
  • U.S. Passport
  • Student ID *
  • Student Visa
  • Permanent Residence / Green Card
  • Employment-based Visa (e.g., H-1B, TN, L-1, EB-4)
  • U.S. military ID
  • U.S. Citizenship ID
  • Immigration Services ID
  • Tribal Issued ID
  • Canadian ID
  • Concealed Carry Permit
  • State-Issued Firearm License
  • Released Offender ID

AlertImportant: If your photo ID doesn’t have a date of birth listed, you will also need to provide an acceptable date of birth document. These include birth certificates, naturalization documents, hospital birth records, religious records, and marriage certificates.

 
Proof of Address

We can accept the following types of items for your proof of address:

  • Mailed letters, documents, or packages dated or post marked within 60 days (e.g., utility bill, credit card/bank statement, paycheck stub, greeting card, etc.)
  • Valid Photo ID that includes your current residential address
  • Current homeowner’s or renter’s insurance policy or statement
  • Current automobile insurance policy or statement
  • Current mortgage statement
  • Voter or vehicle registration card
  • Current lease or rental agreement
  • House deed
  • Current College Enrollment document
  • Current housing accommodations/assignment from college or military base (e.g. Dorm Assignment, Base Barrack Assignment)

Proof of Social Security Number

We can accept the following types of documentation for your Social Security number:

  • Social Security Card, Social Security Card Stub, or receipt from SSA for card replacement
  • SSN from the Social Security Administration
  • Social Security Benefit Statement
  • Medicare Card
  • Employment Authorization Documents
  • Military records with SSN (e.g., DD-214)
  • Federal program documentation (e.g., SNAP or WIC Certification)
  • Selective Service Registration Card
  • Department of Human Services Documents
  • SIN (Canada)
  • Border Crossing ID (e.g., B1/B2)
  • Tax document (e.g., W-2, 1099, and SSA 1099)
  • Loan Application Document from the creditor

Can you accept an ITIN (Individual Taxpayer Identification Number) instead of an SSN?

Unfortunately, we are not able to accept an ITIN.
